These forms are required for integration with our patient payment vendor.

The XML file includes a Study Visit with the following forms:

1. Participant Contact Form: A study visit form that includes demographics of the participant required for payment.
2. Dash Payment Information: A study visit form that includes details for method of payment.
3. Dash Payment Confirmation: A repeated data form that is generated each time a payment is made to a participant.

In order to integrate, all forms are required and all data included in these forms are required.
